How can doctors determine if your hearing loss is due to listening to loud music?

and also i dont have medical insurance.. so should i got to an er or should i make a apointment with a MD and pay out of pocket to get this checked out…?

Hearing came back partially, now in my right ear I have hearing difficulty that varies according to the position I hold my head in, balance problems and a crunching / grinding sensation and noise in right ear when i open my jaw more than halfway.

If you have a high frequency hearing loss and lost ability to hear a bunch of tones, will self-surgery help?
