You will need to take a statement from your doctor with you. When they call on you they will ask you some questions and at that time you will need to produce your dr statement. Have you doctor write a note regarding your hearing loss, and get your boss to write a note saying that you have to work, just in case the hearing issues aren't enough. It's really not that hard to get out of jury duty, at least here in Canada. I'd imagine that it would be similar in the states. Probably. As a citizen it is your duty to do so! Remember, there are thousands of people all over the world sitting in prisons; being tortured and even killed because they want the same rights and privileges as you have. Probably best to discuss this with your hearing doctor or call up the court facility listed on your report form. You don't want to get yourself in trouble by assuming anything.
