What causes temporary hearing/ear problem described “like having water in your ears”?

Dads got this hearing/ear problem that comes and goes for no apparent reason. Its not ringing in the ears. He said its like having water in your ears, where his own voice/breathing is louder. He instinctively tries to yawn to fix the problem (like someone on an airplane) but it never works. Sometimes when he sits forward with his head bowed or lies down the problem goes away.
Anyone now whats causing it?

Must one have ringing in the ears constantly in order to know whether or not there is permanent hearing loss?
